Community Health Needs Assessment & Disease Prevention & Control Measures
Community
Needs Health Assessment (CNHA)
A community needs health assessment by GHEF encourages several communities with marginalized health outcomes to examine the population health conditions, risk and predisposing factors, treatment received, outcomes and mortality in a specific condition such as prostate cancer, pediatric asthma, obesity, hypertension, breast cancer, infant mortality, maternal mortality, diabetes, etc. This assessment allows these sub-populations namely racial and ethnic minorities (Blacks/African Americans, American Indian/Alaska Natives, Hispanics) and the underserved whites to apply a reliable intervention mapping and implementation in health equity transformation.
With public and community health initiatives, upon these CNHA results, elected official and community leaders need to utilize the CHNA in health policy development, implementation and evaluation. Since GHEF initiatives focusses on subpopulations differentials in epigenomic modulation of health (EDH) and with the social determinants of health (SDH) as an integral part of EDH, the GHEF requires the assessment of SDH such as economic, social, psychosocial, food insecurity, exercise, physical activities, as well as the environmental conditions namely, urbanicity, toxic waste, air pollutants, tobacco, vaping, THC, etc.
Disease Prevention & Control (DPC) - Translational
The GHEF requires a reliable and accurate initiative, implying the identification of the risk and predisposing factors in disease incidence and mortality and the application of such knowledge in disease prevention at specific population level. Additionally, there is a need for the GHEF to embrace a translational approach in disease risk factors marginalization and ultimate elimination.
